In answer to your question, I would just write:
Dear Sirs
In accordance with my tenancy agreement, I hereby give you notice to terminate my tenancy with effect four weeks from the date of this letter i.e. x November 2010 [whatever the date is that you will be leaving].
Yours faithfully

Yes the bill is correct, I have been over and over it and tried to find anyway I could to get it reduced (and as I work for a mobile phone company myself I know all the tricks) and there is nothing I can do. And my mobile phone provider are not at all helpful. I will have to pay it though, as that's my only way to keep in touch with my family, I dint have a landline and I would be cut off from the world if I was on PAYG and ran out of credit x0 -
Oh bless you hon! I can't really give you any more advice that what others have said, but just wanted to cheer you on. I know it must be horrible now but one day you will look back and feel so proud of how strong you were and how well you handled this.
Keep posting on here to let us know how you are getting on and stay strong!

Hi guys, I have had a good day today. Did a little bit of retail therapy, and made myself feel better. Hubby is on his way round with my gloves, still no laptop though.. but I'm having a bit of a panic, nothing that I can probably get advice about on here, but I'm having a health scare!! So I'm Hoping to get to doctors tomorrow.0
