Hi Markoo, don't forget to put your reference, your medical/hospital number on top of the letter. The letter is fine although there are a couple of things I would change personally, I hope you don't mind if I put down some suggestions.
You don't have to change anything and I don't want you to feel you need to, so please don't take it the wrong way. Anyway, some others here may disagree with the changes.
I have put changes in bold.how does this sound?
address
medical /hospital no:
Date
Dear Mr ****
I am writing to you to kindly request a medical report in support of an application for disability living allowance which was turned down.
I am writing to them to request a reconsideration and to enclose a letter of support from you would help my case. I would be very grateful if you would confirm my diagnosis, how it affects my mobility, care needs, my likely disabilities and future expectations.
I feel my hip effects me in the following ways
blah
blah...
I feel that it's vital i highlight these things to give a a clear indication of how my hip is affecting me and i would be grateful if you would kindly give your opinion on my condition.
I appreciate you are a very busy man and I'm far from being a high priority but if you are able to find the time to do write this report for me i would be very grateful as i need to send a copy to the department of work and pensions by the 19/09/2010
Hope this is ok, although after reading it back perhaps the changes I have suggested needs a bit of tweaking!:D Sorry if this isn't helpful.
Glad you are contacting your GP, as someone else has said you don't need to see your GP to get a report - remember that they (many) charge for letters or reports - some are quite steep! A letter can be around £25 or more!
However, I think it would be better if you did see your new GP to discuss your condition and how it affects you. He/she may write a letter of support without charging! Also, if you see your GP it may help as it may be done a lot quicker!
When you send your letter of reconsideration make sure you send it by recorded delivery and always keep copies of everything you send.
Good luck.0 -
This is really making me feel ill, i dont want to tell my partner as i dont want to upset her, so im bottling all this up inside, iv not been able to eat or sleep last night i kept waking up in cold sweats, and iv told my other half i just feel ill :-( i feel like crying i really dont need this extra stress i havent felt so low in a very long time.
Markoo, I know it's really hard but try not to worry. It's not for me to say but I think you should tell your partner as bottling it up is going to make you ill.
If something like this was happening to your partner, would you like her or hope she would tell you about it? I know initially it may upset her but sharing you problems would certainly help you.
If she had a problem and didn't share it with you, woudn't you feel hurt that she didn't tell you about it? Wouldn't you want to support her and help her through it? Surely it's something you do in a partnership, you share the bad times aswell as the good - that's what makes you strong.
Tell her, it won't be as bad as you think!0 -
Thanks for all the support everybody, seriously without all of your help i really dont know what i would have done,
I rang my sugery this morning and was told my gp is not avalible untill next week
I explained that i needed a medicle report confirming my conditon and how it effects me and asked if any other doctors would be able to do this for me, she said put down what you need in writing and bring it into us and we will pass it on to one of the doctors.
now im unsure again how to put that in writing and who to adreese it to?0 -
Markoo, if you can get an appointment for next week you will still have about 2 weeks before the deadline your letter of reconsideration needs to be in. If you do see your GP next week he can easily give a letter within a couple of days.
You probably won't have received anything from your consultant by end of next week anyway. I think it would be well worth seeing your GP next week and he will really get a good picture of how things are for you.
As long as you have your letter ready to post all you will need to do is wait for the reports from your consultant and GP before sending.0 -
I would prefer to see my GP in person to be honest, i guess im just paranoid and worried about time but as you say im sure my gp can put a letter together quite quickly, she said hes not avalible untill next wednesday so i guess that stll gives me a lot of time.
Today im going to get my request for a report sent out to my consultant via recorded delivery. He is deffinetly the most important person in the picture as he is an orthepedic doctor and could give a better impression that anybody.
Im just trying not to worry but finding it very hard couldnt sleep again last night and still dont feel like eating. Really dont want to tell my partner would rarther wait until they either tell me the dession has been turned over or its going to a full appeal. she knows i bottle things up and will fully understand why i didnt want to worry hear. i just hope this is over quickly.0 -
Well Markoo, I understand you are panicking about the timeline but seeing you GP next Wednesday gives you plenty of time - Wednesday is the 1st which gives you 2 and a half weeks! Teblephone the surgery now and make that appointment before it goes!
I wouldn't say that the consultant is the most important, the GP is just as important Markoo, the GP is someone who you would see on a regular basis and review your medication etc and will have records from your consultant as they send details to your GP. Your GP will normally be the person who will have a good understanding of your difficulties and needs - if you tell them!
Anyway, good luck. I also understand the reason for not telling your partner but she will have noticed something is on your mind (especially if you are not sleeping and eating!) and will be worrying herself sick about what is wrong!
Please tell her Markoo.;)0 -
Thanks for all the support i really appriciate it! im feeling a little better now, i didnt realise my gp would have information from my consultant etc, thats great news and makes me feel less awkward about going to see my gp and asking him to write a supporting letter evern though i have never really seen him for more than a couple of minuets before.0

No problem Markoo. Well everytime I've been to see a specialist a letter has then been sent to my GP and any test etc the results are sent to the surgery.
Hope you've made that appointment to see your GP next week. Perhaps you could consider sitting down with your partner and have a chat now ...;)0 -
My old consultant used to send letters to my GP and these would be sent to me too. Could you ask for this too? This cost me nothing. (I didn't ask either)Sealed pot challenge #232. Gold stars from Sue-UU - :staradmin :staradmin £75.29 banked

I was under the impression that the criteria for ESA & DLA were different. If this is right how could DLA be refused because of an ESA medical (which are questionable at best).0
