Ok I have actually looked at the form. Section 2 it asks for name address etc then section 3 asks about circumstances at time of PPI being taken out. I guess in section 2 i put lilac pixie but in section 3 because it was before i married I would be lilac elf? Is that right.

Yes I would write the name that you were known of at the time, also make a point of informing them that was your maiden name.
And if the FOS have any of your details, they should give you the account number details, give them a try, or otherwise the lender, you may also find the account number for this on monthly banking statements of where the loan payments were taken out every month, this would be shown next to the monthly repayments.;)The one and only "Dizzy Di"0 -
Been waiting on a claim against Picture since July 2009, this was thrown out because not enough information, thought that was the end of it, but decided to try again July 2010 and found more information i.e. policy number, claim is now waiting on information from new lender. Hope its not going to take long and hope it will go through this time.0

I have the account number of the HP but not the policy number, broker involved so the lender is still lending but the broker well.........MF aim 10th December 2020 :j:eek:MFW 2012 no86 OP 0/20000 -

Yes we contacted them today but still they need more info! Anyway faxed more to them today and apparently they are just waiting on our lender. Not sure how much longer but it has been a long wait - It is sooooo difficult all this waiting!!!!!!!!!!0
