I read in the Mail today that the FSCS had a printing problem with the ISA certificates which has caused delays. Maybe the same problem is responsible for the delay with the fixed term forms? No news yet about my claim using the blank form (barcode obliterated) posted here.0

I've posted a .pdf version of the form for easy printing with the barcode and reference number obliterated here:

Still no response from my form posted (recorded delivery) on 6 Dec.

Despite the reassuring phone message, still no form. It's not that the address details are incorrect either.Hope for the best.....Plan for the worst!

apart from Briskly has anyone else yet sent off a 'barcode obliterated' form? I'm really tempted but don't want to risk confusing the system - wimp or what?0

I get the impression non of the fixed account applications have been sent out so that's a different category as such.
Dorsetlass, I have sent one ( last Friday)
This thread is probably one to watch too:

I've just spent ages talking to the FSCS about this. What I have found out is:
1. Some forms were sent yesterday - the rest will all be sent "next week". Couldn't pin them to a date so assume Friday.
2. Filling in a blank form such as the one posted here is fine - the 6 week processing time will run from date received. No chance of confusion to the system so nothing to lose sending one off now.
3. They have given 6 weeks for the processing time as a maximum - they hope to get it down to 3.0 -
Excellent, thank you for that. I feel much more reassured now and will print off the form asap.0

I have five fixed rate accounts with Icesave, which I opted to leave to maturity. I have received no paper forms yet, though 3 have now reached maturity already. I'm hoping it won't be much longer whilst I've still got a good rate running elsewhere to transfer to for a few more months.0
