Is it clean?
When you want to be sure of high standards turn the plug in the basin over and look at the underside, When you've got an old toothbrush use it to clean the basin overflow then pour bleach over it and have another go. When I pointed this infection risk out to a care home manager she went out and purchased new toothbrushes.
