Amazon debit card unknown transaction

Un-asked for amazon deliveries is a thing that happens. It's a kind of fraud - where the product seller sends unwanted orders to people, then uses that a the basis for fake reviews. Basically it's a way to falsely boost the review scores for products they're selling.
Using a compromised credit card to purchase things (especially vouchers) from Amazon is obviously a thing too.
I've never heard of the two being combined. It would seem like a strange kind of fraud to commit - stealing someone's credit card details, then using that to send them things off amazon. Who would gain from that (unless it's the seller of the mattress protector that did it!)
It would seem a massive coincidence - but maybe the two events are unrelated.

I have checked my bank and credit cards for any other transactions and there are none and also my partner's is the same.
We still have the bed protector she purchased in the house and it still has the Amazon despatch label on it as it was sent out in original packaging.
The item that arrived yesterday is smaller than the item she bought a few weeks ago but as it's in a sealed polythene bag not sure of it's contents.
What puzzles us is how can an item from Amazon be sent to my address with my name and address on it and be delivered by a courier in an Amazon van and I never get any account or email notifications.

What have amazon said when you gave them the tracking number off the delivery label?

Never get that far on chat bot as it asks what item you have an issue with and as there's no order number or reference about the unwanted item chat ends
As far as I can see there is no tracking number only qr codes
Is there an app that can scan these codes to see any info on the package. ?

wait a minute…if its only got a QR code on it how do you know it came from Amazon? Amazon also deliver on behalf of other shippers now, even when an item isn't bought on amazon, they do so for ebay, sites powered by shopify etc. So just because it comes on an amazon van does not mean its an amazon purchase anymore - which would go long way to explaining why you got no notifications from amazon etc.
There are barcode apps for most phones, although using the standard camera might read it too depending on the content. If you post a picture of it on here we can work out where its from.

Was her debit card ever on your account?
Does she have her own account?
When cards expire merchants that have a Continuous Payment Authority in place get given the new number the next time they go to make a payment with the old number. Didnt think Amazon used CPAs but maybe they do these days.
Personally would recommend that you have your own accounts, if you make her "family" in the account then she gains the benefit of your Prime subscription but you maintain your own order history, viewing history etc. We used to share an amazon account but buy very different things, used to be a little uncomfortable loading up amazon in public to have the Mrs' search history on show to everyone.

OP once you click on customer service there are more options further down, one is "Payments, Charges, Invoices and Gift Cards", click this and then the next page has an option for unknown charge.

Her debit card was on my account but expired 18 months ago and was never updated with new card.
No Amazon account ever.
