Getting paid less than others.

Even in the NHS, they can be on different contracts for the same job. Don't let it get to you.
Hospital appointments are not flexible, but you should give as much notice as possible; you need to attend hospital.
Talk to your line manager; it sounds like they are short staffed and they may pay you more if you threaten to leave. Are you good at negotiating?

I’ve only been there 19 month so if i start demanding more money etc they might get funny and try and finish me. 🤔

Did you tell them why & do you need the whole day off?
Life in the slow lane0 -
I was paid less than the person sitting next to me as he joined years before me ,so was higher up the pay scale.
Thst was the conditions I signed up to when I accepted the job.I did more work in a day than he did. I also did more complex work than him at times.
My work ethic was greater than his. That’s life.
If you don’t line it then look for another job.

it does bother you doesn’t it but like you say like it or lump it or move on. Probably be the latter after my week off in September.

You don’t need to tell them why you need a holiday day off.

Youre right, you don't need to tell them, but where it is a hospital appointment or similar it csn help as your manager is likely to be more flexible for that than if you just feel like a shorter week...

You don't need to tell them why, but for a day's holiday they do not need to grant it either, where as there are specific rules around facilitating non-elective medical procedures or diagnostic testing.
You seem to not like your job, why stay when you could earn more elsewhere and it appears improve your quality of life considering you seem to hate it there?0 -
What are the "specific rules" you mention?
