I can't remember where it was first posted though it possibly could be somewhere like Rightsnet. It's not secure but you don't have to input any information. It's a dummy UC account and it's not necessary to input any answers (certainly not personal data) to move to the next screen. It basically presents the options available to someone who may have to make a UC claim, so that the person knows what screens might appear.

Based on your contributions means you have an NI entitlement as far as I know, even if you just getting a IR top up, the full living costs entitlement will still get quoted as it is used to determine the level of top up.If I am correct then after migration you will still have an open NI based ESA claim, but you will also get a UC award, the UC award would be the full means tested award for LCWRA, but then what you get for the NI based ESA will be deducted from it as income, and what is left over with is the new means tested top up.0

IF it also says that it will apply until the end of November that year then that is because the payment will change in December - but only for one payment because the £10 bonus will be added to the first CB ESA payment in December.If you don't see that November date on the breakdown then you don't have any CB ESA and yours is all IR ESA.(I remember that that note used to cause much confusion, with some people thinking it meant that their ESA was going to stop at the end of November).Of course if things go as planned with managed migration then all ESA uprating letters will soon be only for New Style CB ESA anyway.

Is it depending if you was on the original Incapacity benefit Newcad ? that had the payment slip in the book with the added £10 to the December slip. Or benefits office letters some do some don't. I was notified in April will add a draft later.0

Robbie, you might be right about the doc originating at Rightsnet, because at the bottom it states: 'This form was created inside Citizens Advice Caerphilly Blaenau Gwent', since Rightsnet is a forum for professional benefts advisors.
I find the doc very helpful in knowing what the actual UC claim form contains, questions asked, info required, etc, though as Robbie said, the real thing might be a bit different now.

(The migration of those with both IB+IS to ESA they made a particular mess of, a mess that wasn't put right for years).
In fact there are still some people on ESA who still have a 'Transitional Addition' from that IB/IS to ESA migration that hasn't fully eroded yet.0 -
I can't remember the amounts of the age addition payment they varied but some where quite high but im surprised that people are still receiving it. If eventually they are migrated they won't see any increase for even longer. I remember the payment books going because I helped several older people open Post Office card accounts and that didn't last long before it was closed down.0
