B = is details
C = small claims track
D1 = Suitability for determination without a hearing (new question)
E = About the Mediation appointment (picture attached below)

Okay, after much digging I've managed to find an old PDF form which matches the one sent to me by post. And I've answered as per the threads.

Is Birmingham County Court okay for judges? I read somewhere that Warwick has good Judges, I could potentially change it to them as they are on my way for work.

I would choose Warwick, based on some judgments we've seen. You aren't likely to end up at a hearing anyway as that's not DCB Legal's M.O.PRIVATE 'PCN'? DON'T PAY BUT DON'T IGNORE IT (except N.Ireland).
