Thats a reasonable start but you need to add numbered Exhibits etc, so make it more like the exemplars in the newbies sticky thread post 2.  So as coupon mad mentioned, WS and Evidence , She reiterated the mantra earlier in the following thread

A witness statement is actually a bundle and contains Exhibits   ( evidence,  documents,  pictures etc ) as well as the actual declaration statement 

I think that you must have read old statements because your declaration at the bottom is maybe 4 years out of date and now it's maybe 2 or 3 times as long,  similar to the one in your defence submission 

Your WS looks more like a cut down defence,  which you were told not to do, you dont repeat the defence 

So more work and corrections required,  based on studying recent cases with the correct declaration as required in your court order,  so similar to the reply I just gave in the linked thread below

The WS draft shown has not included any of the exhibits carefully listed in the NEWBIES thread (please read the WS & evidence section of the NEWBIES thread again) and it uses a Statement of truth that is five years out of date.

It is very easy to find recent WS to give you inspiration and which have a proper start and ending.

As @Gr1pr says, a good source is to click on and skim-read some linked completed cases in Umkomaas' DCB discontinuances thread until you see a few with a WS draft shown. The majority of those defendants had drafted a WS in their thread for all to see, just before the discontinuance arrived.

There are over 575 completed cases listed there!
