Ah I see. I think you just mean the N180. The Directions Questionnaire. Not a mediation form, as such (there's no such thing).

£15... ouch! As @g1rpr says, the first 8 steps in the Template Defence thread covers every question on that form and you can just email it, we also provide the email address there in the 8 steps.

Nothing goes to Euro Car Parks direct anyway. Claimants use solicitors for a reason and everything for the Claimant goes to DCB Legal.
