Rogue's template is all you need - the important bit of the SAR is the bit listing out the personal data that you want (or simply saying "all" personal data). Headers specifying individual clauses and paragraphs of the DPA or GDPR aren't important - if you want the recipient (and assuming its a small company without a legal department?) to have further information or guidance a simple link to the ico website will be far more useful to them than technical legal language.
If you really want to ref the section of the DPA dealing with SARs it's section 451 -

The courts tend to dismiss these recordings as hearsay unless they are corroborated. The corroboration they look for is from the called party to say they have checked the recording against their own record and it has not been edited, etc.0 -

For what it's worth the reason I mentioned HMRC is because it was exactly their denial of a phone call - resulting in huge problems for us - that has destroyed my trust in phone calls as a means of passing on important information. No other reason.

Can you select the email address the request goes to? If so, put yourself in there, and receive the request by email, then copy/paste/edit/print as needed?GaryBC said:
