It's not "The Quest" (as in "It's in all the questions").

Edit: I re-read it, and it says "It's all in the questions" so I used a guess for nothing....  :#)

I suspect it won't be as simple as answering all the questions and reading the first column - it never is.
There has to be a reason for needing to print the form, though I'm not sure what, but it's part of the T&C.
    • I wondered if "To remove a limb" might be DISARMED.
    • I think TRACTOR may be too easy

What if we have to associate the questions with time prepositions at the end on, at, in, from, to, for, till, by.. etc
  • There could be words hidden in the questions like touch having ouch in it

i think Its a trick, "The answers are in the question", 3 blank spaces so who has a name that is 3 letters long with a second name that's 5. I could get the first name from the first 3 questions contains the correct letters but for the second it doesn't add up, possibly my best fluke ever?
For those who want a clue, the person plays with an egg shaped ball.

*Edited* its not a trick i got his last name from my spreadsheet of answers, dumb luck wins!
Possibly the greatest clue ever, kudos to you
  • Still a bit confused as to how you get the letters from the questions but I have now got the answer
