Fit note is dated until 21/11.
You said "I've also just had an appointment with my GP. She is giving me an indefinite fit note dated from today".

If it's for the next 3 months that's not indefinite. You'll need to keep getting it.
Yes I did say indefinite because that's what she told me. At that point, she hadn't emailed it over to me so I didn't realise it had an end date. With it having an end date, I realise that I'll have to keep getting it.

Looking up the AET, I'll still be earning over that.

The AET increases from 26th September to £494/month (for an individual).
Information I post is for England unless otherwise stated. Some rules may be different in other parts of UK.

Thank you @calcotti, I'll still be over that. I'll be on around £520/monthly.

Just be aware that any month your earnings drop below £494, you will automatically be booked in for an appointment at the jobcentre. If you are unable to attend due to work, make sure you tell them (in advance, and again after failing to attend) that the reason you were unable to attend was because you were working.
But hopefully it will not be an issue for you if your earnings are always above the new threshold.

Hopefully that won't be an issue. Earnings shouldn't ever drop below that, company pays for being off sick as well so unless I'm off long term sick, it should be ok.
Good to know though, thank you.

.. I report a health change on my UC and upload the fit note? Then I just leave it and they'll be in touch with any questions or forms that they may want filled in or do I have to actually apply for LCW?
You should be issued the UC50 29 days after you report the health condition. If you don't receive the UC50 after a month you should chase it.
I've received the UC50 this morning, I'll get that filled in and sent off.

There's a guide here to help complete the form. Although it says ESA the form for UC is exactly the same. https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/employment-and-support-allowance/help-with-your-esa-claim/fill-in-form-esa/
You need to make sure you send evidence with the form to support your claim because they rarely contact anyone for any evidence. Also make sure they receive the form back by the date on the letter.

    Thank you Poppy.
What evidence? I don't have any, all I have is a fit note stating what is wrong with me.

Maybe you can get evidence from your GP - a printout summary of your medical conditions, list of prescription medications, copies of any letters of referral to consultants. Going forward you should start to collect evidence - always keep letters for any medical appointments, scans, tests, consultant visits etc, and ask the consultant to copy you in when writing to the GP etc. It all helps.

A fit note is not classed as evidence. The same for future appointment letters. You can also include a couple of real world incidents of exactly what happened the last time you attempted each descriptor that applies to you. Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.
Some people find it helpful to ask someone that knows them well if they can write a letter of support. Make sure they include detailed information about how your conditions affect you.
Remember, you will be assessed on your capability to do any type of work.
