    Download your own copy from MCOL, fill it in and email it back to CCBC (and the claimant).

    Sorry I thought the claimant would send me the directions questionnaire to fill in. I didn't know you had to download your own.

    Why did you think the Claimant would send you your DQ?
    Item 8 on that list clearly states that the CCBC will send it to you.

     You don't need to download your own.

    Why have you given up on that list?

    Here's the next item, item 8, from that list...
    I strongly suggest that you read items 9 and 10 from that list too.

    Did you receive a N149A along with the N180?  If you did you will likely find a deadline date on there.

    No I haven't received anything other than the claimant's directions questionnaire. And the MCOL has just been updated today with the "DQ filed by claimant and DQ sent to you"
