I would fill it in as soon as you can. When does the current PIP run out?

They have extended the returns of all forms to 3 months from the date the letter was sent to you. When ringing DWP/PIP there's an automated messaged telling you exactly this. I also received my review froms, which were sent to me on 2nd March, so mine have to be returned by 2nd June 2021. You can of course return them sooner if you wish. For confirmation of this then just ring the PIP number 0800 121 4433 and hear it for yourself. I double checked again this morning.

Thank you very much for the info. Is there a reason why the date was left blank?

Most likely because of the 3 month deadline to return the form. The backlogs are huge at the moment and reviews are way down the list as are those transferring from DLA to PIP. Their focus is mainly on new claims and has been all through the pandemic.

One thing i didn't mention with the 3 month extension to return the forms, you will likely still receive the reminder letter to return your forms within the first month but just ignore it. It's automatically generated.

That will be for the same reason as mentioned above, there's 3 month timescale to return your completed forms and all supporting evidence.
  • Thinking about this there really should be a date on the form. As how can there say the form is late if there is no date on the form. Regardless of what it says on the phone message.

There should be a date on the letter that was sent out with it.  Presumably that would be on the system.  I suppose it maybe causes less confusion/anxiety than having the wrong date on the form - though that assumes they put the right time frame in the letter, of course.
