OP, you need to add the following to the end of your GF's defence: -

I believe that the facts stated in this defence are true.  I understand that proceedings for contempt of court may be brought against anyone who makes, or causes to be made, a false statement in a document verified by a statement of truth without an honest belief in its truth.

Correct, that is what you do.

Make sure you add the claim number back in, and that nothing remains in red.

Then read bargepole's summary of what happens when (from the NEWBIES THREAD) and how she should fill in the DQ (next form).  We don't have time to answer questions about the DQ, like 'should we agree to Mediation?'  NONONONONO!

Stay in touch though and read witness statement examples to see what will be needed by way of evidence, later.  Go and get some photos in the dark when on a walk, if this is local.

yes , add those missing details in , get your good lady to sign this defence , get her to email it to the court as a pdf attachment and attach that appendix too , to hope that it gets thrown out early on and to put the fools at OPS on notice that the GF is no pushover

remember , they havent paid the court fee to Brighton court yet, after the DQ is done it will be months before any court hearing, many months , if at all

She can sign a piece of paper that you photograph then email it to yourself, crop it then plonk it in...
