Perhaps he’s right to bring some balance? The key word in your sentence is almost, which most posts seem to ignore. And, I hope posters are always honest, without needing an exhortation!

Given that I spend a fair amount of time in Spain I find it difficult to understand why one would return to the UK solely for medical treatment. - there are almost certainly better ways to organise things unless the landlord is a Spanish resident who speaks no Spanish.
And we are told that HMRC are aware of the landlords address.
If the landlord is cheating it seems to be rather half-hearted cheating.
