If you're drafting in Word, take a photo of the phone bill, then insert it into your Word document.
Please note, we are not a legal advice forum. I personally don't get involved in critiquing court case Defences/Witness Statements, so unable to help on that front. Please don't ask. .

Does it have to be the exact same transcript or is it ok to transfer the file to text and use it that way?

No it must be the approved judgment transcripts.  Can’t you use a PDF merge tool?
PRIVATE 'PCN'? DON'T PAY BUT DON'T IGNORE IT (except N.Ireland).

Yes that’s the Chevalier Firescu case.  You should be able to merge your signed and dated witness statement with your transcripts one after the other, and add page numbers to the whole lot.

They have provided a photo of the signage, £100 in tiny print on the bottom right. Can I use this in my ws and refer to their witness statement or add this as an appendix in mine?

Can I add this to my WS do you think?

I was under the impression the original PCN was stating that I had not paid for any of the parking period. According to the claimant's witness statement they are claiming I overstayed as they have a record of my paid 'permit' of 3 hours. This evidence was omitted when I requested a subject assess request. What perplexes me is that my bank account from that period shows no payment to RingGo, despite Premier Park having a record of the payment. Regardless of whether I overstayed or did not pay at all, the sum cannot exceed £100 as per the signage (in very small print) and according to POFA as the original PCN demanded £100.
Also thinking of adding this to my expenses part:

    1.     As a litigant-in-person I have had to spend considerable time researching the law online, attempting to interpret correctly legal terminology and preparing my defence and witness statement. This has been challenging and stressful in itself as I am a working mother of three young children, working as a nurse through the current pandemic. It has also been an emotional strain regularly receiving threatening and harassing debt collection letters. This has been acknowledged by Lord Hunt of Wirral – “Highly undesirable practices in the private parking industry range from threatening letters sent to motorists, poor signage in car parks and aggressive debt collection practices”.
