    It is semantics sangie.

No, it is not. Semantics is asking (or answering) the same question in different ways, using different words. You however, are answering a different question.

Whilst it might be that the question you are answering logically follows the question you should have answered, it is entirely sensible to answer only what you have been asked, and not volunteer additional information which might be detrimental to your cause.

    Therefore:

Q - have you had any periods of sickness in the last year?
    A - yes: a 5 month period.

Not: - yes: diagnosed with mental illness & signed off work for 5 months.

A common interview tactic is for the interviewer to ask a question and then simply shut up. Or leave a loooooong pause between the end of an answer and a follow up question. The nervous interviewee feels awkward and needs to fill the silence. By rambling off a tangent, usually digging themselves deeper and deeper into a hole. This question is similar. Volunteer only the information you are directly asked, no more no less. Yes, 5 months.

I know i got myself into a mess but it was a face to face question and I completely froze up and lied
Are you lying again? Because in your OP, you say that you filled in the form saying you had no sick days.

I think the best you can do is contact your manager, explain that you lied because you want this job so much because it's so perfect for you and that you were worried that you would be dismissed from being given a chance just because of your sickness. Say that you've been to the doctor and you have now started treatment and are feeling a lot better (assuming that is the truth).

Say that you feel bad about the whole situation, hence calling them to come clean and discuss.

Are you able to evidence that before April, you've had few days of sick for years?
