here is the web site of a company I have used before with success. they are based in North London but operate a postal service for BB5 phones, unless you live in North London, in which case you could visit!

Here's the address, http://www.gsm-support.co.uk/, the charge for unlocking is very reasonable at around £5. BTW, I don't work for them.
