According to this link, it seems to be a term that could get you into trouble if someone takes it the wrong way: :D

Apparently it's just a variation of BFF.

I only ever say 'you're welcome' to someone when they thank me if I really mean it.

I say it quite often when someone thanks me for a gift I've given to them.

Yeah, I'm sure many do. If I said it myself I'd mean it too. If saying it in person you can usually tell how someone means it.

It does, especially in writing, have the possibility of being taken or meant differently though.

I'm the exact opposite. I've never heard anybody say thing.

Thing doesn't sound correct, nor does it make sense.

This source acknowledges the alternative use of "thing" but suggests that it is the result of "think" being misheard.

I'm in the 'another think coming' camp, too, as it's simply playing with words. It means 'think again' and the grammar is irrelevant here.

And that's from an English teacher. (Retired) :p

So what is the second B?
(Assuming we're using the Best Friends Forever version, rather than the alternative :D)
    Best B........ Forever

It's become relevant in the sense of it's part of the reason it has continued as thing. It's still irrelevant in terms of what it means.

I often feel rather awkward in them situations though, especially if people keep thanking me over and over.
I get that sometimes at work. Over-effusive thanks for doing something which is part of my job. I mean it's nice to be acknowledged (not taken for granted) but "Thanks for that" is usually enough. If I've gone out of my way to prioritise your task over someone else's or done something extra - great but some people feel the need to then tell me that I'm being really helpful and it was just what they wanted... when "it" is the spreadsheet that I send them every month, in the same way at the same time, because it's my job to create that spreadsheet. I'm finding myself trying to make a joke by saying "It's OK - I get paid for doing it..."
I need to think of something new here...

I can't stand the way the word "sad" has been hijacked as "youth speak". It should refer to an unhappy event or mood, not having age inappropriate trainers on.

Well technically it still is referring to an unhappy event.

For that example, "why would you wear them trainers, they're kids trainers! That's sad." It's sad for them because they've got to walk around feeling unhappy (or not as happy as they were) because their friend looks an idiot in kids trainers and that reflects on them.

"Them trainers are sad" or something along them lines would have the same meaning, it's just worded stupidly.

Edit to add: It can also refer to the other person being sad. e.g. "You wore the wrong trainers? That's sad" because they think/know the other person is unhappy at having accidentally wore the wrong trainers.
