Okay I have found a thread with same situation as me with appeal samples.
I will take that and make my own and I will post here for you guys.
In the meantime I am posting the letter I received from CPM.
If that helps.

Please add "http" as the site is now allowing me to post links (new user).
Much appreciated.0 -
Okay a little update.
I have called UK CPM and told them the code on the letter is not valid.
The lady then gave me a new 10 digit code.
Now should I inform BPA about this as they asked for the copy of the letter (sent via email)
Many thanks0 -
This is really silly.
The new code given to me is not valid either. Can you believe it.
I get following error when i enter that into the POPLA submit appeal.
Verification number invalid - please re-enter the code. If it is not recognised please contact the operator who issued you with the verification code.
Do I call them again or let BPA know about this again ?0 -
Okay so I rang them up again.
I asked them to give me a valid POPLA code as the previous one was not correct.
So I am now given new code, which I inserted on the site whilst I was on the phone. And apparently this is the correct one.
The lady over the phone insisted she gave me correct code.
Even though they are both totally different no way close.
Anyway, what do I do now ?
Go to POPLA site and submit appeal online ?0 -

Yes, as per the Newbie sticky.
'Belt and Braces' Also post via snail mail and obtain a free certificate of posting from the post office.
Also complain to the BPA and DVLA about the trouble you had extracting a valid POPLA code from the parking company.:j0 -
Don't just blithely file a POPLA appeal without first getting it checked, unless you are sure your appeal is good. (Remember - mitgation does not come into it AT ALL).0

What do you think, do I have any ground here ?
I mean i have a permit, i was parked in my own designated bay. The permit just fell off due to condensation etc.
I have pictures of people parking illegally infront of my bay and leaving the car to go collect their kids from the centre because our car park saves them a walk.
They are never given tickets.
And I have trouble reversing into my bay, as there is no room to manoeuvre.
Please tell me can I defend this ?
You are all experienced.
Couple-Mad. Need your expertise.
