Definitely send with some form of proof they received it.
You might find it difficult to do as it emphasises all the things you struggle with but keep going.

Thanks Beanie - it is horrible the way it focuses purely on what you can't do or struggle with or take twice as long as someone else with or experience pain and breathlessness with.
