Yes, but to varying degrees of certainty (100% certain, vs strongly suspect vs never stated it as a fact vs distinctly remember). I questioned which is was and all I got was an evasive reply. If really certain and distinctly remember, then it is highly likely she noticed at the time, which would add a lot of weight to her argument. But if she noticed at the time, why wait until later to query/complain? Perhaps she didn't wait or perhaps she did and there was a valid reason. If she didn't notice at the time, can she really be 100% certain? etc etcThe OP has remained adamant throughout that she didn't sign anything

further observations:
OP continues to refuse to say which bank it is, which denies us knowledge of their normal withdrawal process.
OP has not said if the bank have produced the alleged forged slip, or where they are in the complaints process.
OP has had more than a normal lifetimes-worth of problems with her bank, multiple mistakes, lies, complaints, past compensations, yet perseveres on a lone mission to reform and retrain them; that is not rational behaviour.
OP rants and raves a lot.
