No not all of it, bit from Summary of the claim: ending with the appendices was added into the detail bit, the other information above that was just pulled from the confirmation email they sent through to me.
the actually Appendices were sent via email once i'd click submit
Did you send them the emails you sent them too? Or just EE's replies?

I sent them the emails too and from.
I had a small claims court case a few years ago, and my old landlord (who was suing me) forced our letting agency to change a check out agreement, but foolish had copied me into the an email, he omitted that email from his email chain. if i hadn't submitted all emails that might have gone unnoticed but as it was played in my favour.
If you copy everything in that you've sent and received, you bring into question why EE or whoever (if they have done) have missed out elements, which brings into doubt elements of the case, or at least that's my reasoning anyway.0 -

I'm with EE proper and this is what i submitted on the basis that i only left in the paragraphs that we relevant. so in the original template I only had one paragraph that applied to my case

Summary of the claim:
My claim is not in regards to EE (referred to as EE throughout the remainder of this claim) business decision to increase prices, as a business EE is free to make whatever business decisions it chooses. My claim is that the price rise notified to me is of material detriment as per the Ofcom definition of Material Detriment under GC 9.6, and therefore under GC 9.6 I am entitled to a penalty free cancellation.
Further I request that I be awarded £219 compensation (£73 for each made up of 2 contracts 1 x £41pm and 1 x £32 pm) of the following:
• EEs breach of GC 9.6 – not informed of my rights to cancel the contract penalty free should I consider the increase to be of Material Detriment, (£73)
• EEs initial lack of response to my email of 15 April 2014 – taking 5 days and 1 follow-up email until a response was received (£73)
• EE falling to address the points raised in my original email even though I had made it clear that a response that did not address each point would not be an acceptable response (£73).

Im at the stage of submitting a claim to CISAS
Can someone who has done this give me a few pointers?
Ive had my contract since June 2012, so its nearly up.
On the template Page 1 it has
I take it i delete the first paragraph and leave the 2nd one but remove the part in brackets and in red?
Then at the bottom of Page 1 it says :
I realise Appendix 2 is a copy of my correspondence with EE, what do I change the red X to?
And lastly , because i have a pre 30th Oct 2012, i just leave the blue text in on Page 5 and remove the green text on Page 6?

Yes you have got this right.
Appendix 2 - X = the emails (to and from EE) if you put all of the emails into one word document then you will only have an Appendix 1 and 2. If you put each correspondence as a separate document then the number of appendices will = the number of file PLUS 1 - which is the main claim.
The part which you copy and paste onto the CISAS application is headed "Summary" all the way down to "Appendix 1- X" - removing the bits that don't apply.

You need to put the following in the section "Take some action" in the CISAS claim form.
'take some action' - Give a back dated penalty free cancellation and a PAC and unlock code
It should be obvious from the correspondence and case that this is what you want, but the adjudicators can be sticklers for procedures and rule that as you never requested them to ask EE to take any action then they can not ask EE to take action regardless of the merits of your case.
If it is not too late I suggest you contact CISAS immediately and clarify this with them.
As an example on the T&C forum one person had 4 contracts but as they only referred to 3 in the claim document (even although EEs defence referenced all 4 numbers) the adjudicator stated that they could not make a decision on contract 4 as it was not in the claim!!!0 -

i just submitted my claim on the CISAS website (i use chrome browser)
I entered in all the details, added a couple of attachments, clicked submit and got this errorNotice: Undefined index: Explanation in /var/www/vhosts/cisas.org.uk/httpdocs/inc/application.inc.php on line 24

If I remember correctly you should get an email (auto reply) with a case reference number. If you have not got that then try again - or call CISAS.
I recommend that you send the attachments (evidence -Appendix 1 - X) in a separate email as attaching them with the claim seems to be a bit hit and miss.0
