An electronic version of the questionnaire is available on the DWP website. This would still need to be printed off to be sent in, but it can be typed into first.
I strongly advise people first do a 'save as' of the ESA50 pdf document to their computer or equivalent storage device and use that form before typing. If one enters information into the pdf on the website then you won't be able to save changes (unless you of course do a save as at that time) - and I would assume many claimants won't complete the form in one session and will want to keep a 'permanent' copy (quite possibly to make completion next time around less demanding)."Do not attribute to conspiracy what can adequately be explained by incompetence" - rogerblack0 -

Why are HCP's not fully identifying themselves to the claimants? i.e. if the claimant asks for the number they have been given by the GMC/NMC or other medical body the HCP is registered with why won't they give out that number, HCP's claim they do not have to give out their registration numbers when asked for it. When the claimant then checks the name on the appropriate site they then get a list of names matching the name given at the assessment in some cases.Someone please tell me what money is0

Our health care professionals take into account all relevant information that is available. It is important to remember that, although all medical information is useful to the healthcare professional, it is the elements of medical reports that refer to capability that is most significant.
None of our healthcare professionals are on commission.
THEY DO NOT ALWAYS DO THIS! (First time I've ever raised my 'voice' on MSE!) For example they sometimes do not read the ESA50 (obviously relevant) they have in their possession prior to the medical commencing as they do not have time to (or can't be bothered). How many times do you need to be told. That's why I've asked the question I have... what does the claimant (customer) do when they find the HCP has not studied relevant information as required to do and are in a medical assessment situation? If you do not know the answer, just say so and I'll stop asking. I'm not here to make you look silly, I'm here because I want information on a subject it appears is well within the bounds of what is set out at the start of the thread."Do not attribute to conspiracy what can adequately be explained by incompetence" - rogerblack0 -

It would be interesting to see how often the Pre-Board Check system is carried out and how often ESA113 forms are issued!0
