    You can find the form by googling it. You have to have a code though The original forms don't have codes on them .

    It is a proper printed questionnaire which cannot be photocopy nor there is any code on it.

    Still waiting for my pillow to arrive, but there definately was not a code or reference on the questionnaire, and like others above have stated its not a questionnaire that can be photocopied, its on thick paper which folds into an envelope to be sent back

    I've had no luck so far in finding a Dreams store that actually has the questionnaire in stock! Still, the offer goes on for another month, so I live in hope.

    Just back from my local Furniture Village, had a demo, nearly feel asleep at one point, so comfy!

    Quote of about £2500 for 4 drawer divan with original 20 matress, mattress protector and 2 pillows, so not as bad as I was expecting I must admit!

    Gave me my form to fill in, I completed it while he calculated how much foir the devan ect (My request, not his pushing) and he said they'd post it for me when they took their post to the Post Office, very nice and cizillised!

    I got my pillow today and my nan got hers yesterday. WOW I love it , its so soft and compfy and a good size for a travel pillow. Thanks so much to the op, one of the best freebies I've had.

    I filled in the questionnaire online and just put "N/A" for the code.

    Wasn't expecting to get a pillow, but surprisingly got one in the post.

    I've had the people in Dreams tell me the form is online & you don't need a code, thanks for a wasted journey :mad:

    If anyone has a link for the form, I'd very grateful :)

    Have tried searching for it & can't find it anywhere...
