I have an email stating that switching forms had to be in by 1 Nov to qualify. My actual switch took place on 15th Nov however. I think Dr_Cuckoo3 is referring to the date the swich form was completed.

yes - that's correct

also I was an existing account holder - so it was not a new account and the switching form was submitted online

got the £25 from nhsdiscount but not the £200 yet. Wonder whether it will be paid later. completion date 14/11/11

I got an email from them on the 24th October thanking me for completing my switching form. I think, from memory, the switch was fully complete on the 14th November.

No £200 yet.

Same here, £25 from NHS Discounts received today.

Completion date was 24/11/11.
