It's always possible, indeed quite likely, that the caller who took over your mother's account did provide additional security answers. They must have had her name, address, and bank account number. It's possible they also knew her date of birth, or details of other bank transactions (from a statement stolen in the post or from her rubbish).
If someone did have sufficient information to take over your mother's bank account then she is a victim of an identity fraud, and there may well be other activity going on like credit card applications, loans, etc etc. I strongly recommend she should get a copy of her credit reference file from Experian and Equifax to see if there is any untoward activity. I would be VERY surprised if there isn't!
Stop ranting about the bank, it is them that will suffer the financial loss regarding you mother's bank account, and they are victims in this matter as well.
The reason people are suspicious on the forum is because they asked pertinent questions and you didn't answer them. For example, how did the thieves manage to withdraw money? When you don't answer those simple questions, and describe peculiar behaviour from the bank, people will challenge you.

Lith has made an excellent impression on me since his glorious debut here a week or so ago.
I honestly cannot wait to see more posts filled with his sparkling wit, expert advice and pure gravitas, all delivered in the Queen's perfect English. Seeing his name on the thread list makes my heart skip a beat as I dash to see what wond'rous and insightful things he has newly bestowed upon this fine forum. Although he has only just begun what will surely be a fabulous, glittering posting career I can only imagine his brilliant prose will lead him to glory, to being a well-respected and much-loved member (or should I say "elder"?) of Martin Lewis' pride and joy.

NO ONE CAN CHANGE AN ADDRESS OVER THE PHONE IN THE CALL CENTRE WITHOUT A SECURITY NUMBER OR PASSWORD!
No one! Not even a manager, senior manager or call centre manager. NO ONE! There is no magic button to press to override the security!
Even if the customer rings up and says their house burnt down last night and they want to redirect the post to a new address, if they don't have a security number or password, the adviser will say I'm sorry, please write in or go to branch, thank you. That's it!
Something is not right with this story.
Why does it take someone 3 months to notice their statements are not coming?
Why come on to an internet forum and not have the full story and then shoot people down when they give advice?

Well... i don't know the full story nor does my mother till she finds out what address this is etc.. 0_o otherwise i WOULD tell the full story... im just telling what i KNOW
