is it normal for another GP to fill DLA form in

i applied for dla and was refused
i asked for the evidence they used maily the DR report as i wandered just what he had said that made the DLA come to the conclusion they had

the doctor that filled it in was not a doctor i have ever seen in my life, i presume they just sent the forms to the surgery and who ever was free filled it in

i have appealed it on this basis explaining that i had never seen him and if my own doctor had filled it in i believed that the answers would have been different

im undergoing various test to confirm or rule out things this was not put on the form alot was missed out

this other doctor though highlighted my MH alot, it suprised me as i was not really claiming on these grounds as i cope but on my mobility

i sent off a report from bupa and an independent occupational report, which i hope will confirm my mobility is poor and supporting the care i need although i have had a lot of time off work, they have now spent a lot of money altering my working enviroment and purchasing a erganomic chair to help me stay in work even though i am doing half my hours

will the DLA re-wright to my doctor?

    i have similar prob exept my doctor did fill in the frorm but missed out like 90% of the illnesses ! he even held up his hands ..me very bad ! ...he said to write to them like you , and if they send a note back ...he will fill it in proper this time ! i have heard of doctors signing the form and secretaty filling them in ....i had a report that i paid for , which she did for me which was hopeless as the doc had losted his notes , and she just added bits together which were not right , i had to go in and get them to re-do ..no wonder people give up !

  • I cant repeat often enough that its a good idea to see your GP at the time you are applying for DLA or a renewal,update them on your care/mobilty needs and let them know they will be hearing from the DLA people
    if like ours your surgery is fully computerised then anything and everything you discuss should be put onto the system

    I don't understand? Why should that be necessary?

    Surely the report is completed from the historical info held in the practice and can be filled in by any doctor using those notes. Are you actually saying that you do that to make sure the GP will remember what to put on the report? Presumably you believe he won't bother reading the notes.

    If so, your GP sounds like mine. Except that I have given up reminding him of things - that isn't my job! He needs a damn good secretary!!

    I worked for 12 years in a 3 GP Practice and I never saw them fill one in. You have to fill them in or get help from Dial or a CAB officer, cos they are a minefield. I am sure they make them so difficult and confusing so that you wo't bother with them. I filled one in for a friend who ha d had both legs amputed, and to each question about walking andmobility, I just put, I have no legs. He was awarded his DLA.

    When DLA contacts the GP for a report it is a different document to that of a DLA form that you get when you apply. It is sent to the surgery and can be filled in by any one of the GP's at the practice - it doesn't have to be your own GP.

    One time when I re-applied for DLA, I was contacted by the surgery asking me to attend an appointment with a GP I didn't nomally see - the practice had received a DLA form - REQUESTING MORE INFORMATION. When I attended, the GP (which I had only seen a couple of times) asked me a lot of questions, I was examined etc. He was making notes AND looking at the computer ... probably taking information from my previous visits with notes by my usual GP. I had received a letter from DLA stating that they had contacted my doctor's surgery for more information.

    Although he wasn't my own GP at the practice, I was awarded DLA.

    THE FORM THE GP RECEIVES IS NOTHING TO DO WITH THE FORM YOU FILL IN WHEN APPLYING FOR DLA. Your GP does not fill in your DLA form.

    I'd like to know who goes to a GP practice that is small enough for a doctor to know a single patient well enough to remember things that are not in the notes?

    What may be more worrying is pertinent facts that the doctor 'remembers' but are not in the notes for other medical staff to access.

    or you have your own doctor who knows all about you, and one appointment with another who tells you to your face you don't have MS.

    I was so upset by her, that at my next neurological appointment i told the dr what the gp had said, who wrote to her, i haven't seen her since, gone off her, how can she possibly know i don't have MS, she is a GP, she does not specialize in neurology

  • The form your GP completes and sign's is called DLA-140 and I posted an actual example on this board last week. The advice he must follow is an updated version of this one.

    Sorry about your experience regarding your GP ...especially when you have been referred to neurologist who has stated otherwise!

    This is why you have to make sure that you write and give full details about the specialists, test results, etc on your application for DLA. If you wish get reports from the consultants you see and anyone who cares for you to send with your claim. There is plenty of opportunity to write on the application form what you wish ... it is noted at the back of the form that you may add further pages.
