there are calcs that can generate the codes (see 1st post of the sticky) but sometimes the settings are wrong or nokia block some settings so we have to keep notes of known working settings as well

Insert a WRONG sim card, one that doesn’t work in your phone. When it says phone restricted, press ‘undo’. It will ask you for a restriction code, and input the code into the box on the screen.
