There are a lot of questions, and we haven't found any guidance on this query.
We will probably use the name on her passport, but it would be good to know they definitely don't use birth certificates. I couldn't see a space to add explanatory comments.
    Thanks.

Just to update, we used the name on the late mother's passport. Also, I was unclear on using 0044 or +44 ahead of our UK phone numbers (I used 0044) - the help ? pop up didn't address this - and both applications were accepted almost immediately. Thanks.

You can complete the basic information (i.e. passport info etc) without having to enter details of where you intend to stay.

I recall that it states that somewhere or there are * against the mandatory sections where passport details are required and the lower area of the screen can be left unfilled.

After all, if you intend to travel to the USA more than once in 2 years then you would need to be able to change the point of contact info for subsequent visits.

A quick look at the form online shows that it doesn't ask about previous ESTAs

Possibly a silly question, but when asked for my mother's surname/family name should I put her married one, ie same as my father's or do they want her maiden name?

When we did, used married name. No issue.

  • Hi Everyone. I applied for a ESTA in June 2013 and I am going to Florida in May. My ESTA is still valid and I have updated it with the address of where I will be staying. However, i cant update the flight number and carrier. If my ESTA says I'm flying to Newark but this time flying to Florida will it still be valid? I'm worried they won't let me on the flight. Should I apply for new one or do you think it won't matter?
    Thanks in advance.
