    Yes, I wrote them a letter with our National Insurance numbers on and told them when we'd moved to Spain and asked if we were entitled to the E106 or E121 to entitle us to Healthcare.

    I got an E106 because I had paid enough NI Contributions immediately before leaving for Spain; my husband got the E121 as he is on IB.

    When my E106 ran out two years later, I rang them and they added my name to my husband's E121 as his dependent.

    And yes, you do have to let them know you have gone because if you are Spanish resident (which you are automatically if you live there for longer than six months a year), then you are no longer entitled to British NHS care other than for emergencies sustained when you are actually in the UK.
