New 4 Page DLA Renewal Form( DLA580)

I recently received my DLA580 Renewal form Which I attempted to fill in myself I was told later that I shouldn’t have put some information down about attending a CBT course to help with my condition it could be conceived I was getting better when the reverse is actually the truth .Someone from MIND rang the DLA up & told them I spoiled my form & could they please send me another form ?

When the form arrived however it was the old DLA1A claim form so MIND rang them back to inform them they’d sent the fresh claim form not the DLA580 renewal form
They told MIND that the DLA850 form doesn’t exist & that the DLA1A claim form is the correct form to fill in
Now I’m really anxious & freaking out because the present climate of trying to reduce people that are on disability etc that the DLA people are actually given me mis information or trying to get me to fill in the new claim form DLA1A instead of the correct renewal form the DLA580 so I will be judged as a new claimant & easier to refuse instead of a renewal ..another issue is I now have two return forms bith with the black printed DLA address but one as team U.2.2 3209 written above the address but I can’t remember which envelope came with which form .

I noticed reading some of the messages on here that the DLA580 was on a trail period in some areas does anyone know if this is still the case if so which areas[I’m in West Yorks] if not is the DLA580 the standard renewal form now ?

I'm in West Yorkshire and I got the 4 page renewal form, well we're calling it 4 page but basically it was tick one box and sign. I've since spoken on the phone with them about a change in circumstances so my case is not straightforward, but if someone gets a change in benefits but no change in circumstance than perhaps it is based on a new medical report.

I have my renewal forms, award due to run out in November.

I have the long version.

Can anyone tell me if they have access to the ATOS medical results from ESa medicals?

I have read on here of peoples DLA being stopped because of their ESA medicals with ATOS.

I dont have a problem with them having access to them, i felt my medical was fair, just wondering if its true.

You put everything down - not just that you're getting worse.

yes they share info with esa/atos.

my mid rate dla care was awarded on the grounds of my esa medical - so i didnt have to come in for any other medical.

they even wrote in the dla letter that they had made the decision based on my medical (and writing to my gp)

so in my case, it was beneficial.

Dark White, Im in West Yorks, way back in january i received my renewal form, it was the 4 page version, sent it off, waited and waited, when my CPN rang she was told they'd never received the form, it was lost in the post. So they sent a replacement, this time it was the massive form, she rang and said it was the wrong form, was told the 4 page form wasn't being trialled in West Yorks (despite myself receiving one and my partner having his award renewed using one). So filled that in and waited, came back a while later with a reduced award. So i asked for a reconsideration, only for it to come back the same. Turns out they haven't received the Psychiatrists supporting letter, its lost in the post. Don't know whether to laugh or cry.

What i would say is to watch the dates on your renewal, mine wasn't due to be renewed until the end of June, but they have awarded me from April which is when i resent the form. ATM my CPN is concentrating on getting my HRC reinstated, but will be questioning it later. Also, send everything tracked, both my CPN and Psychiatrist sent the two that have gone missing normal post, whereas i sent my 2nd claim form and reconsideration special next day delivery and i knew they had been received, .

I really don't think the DLA are trying to trip people up - no, i'll rephrase that, because i totally understand why you're anxious and freaking out and i dont want you thinking im dismissing what you feel, when im able to see things logically, which takes a lot of persuasion and effort on the part of others, i don't think they're trying to trip us up - but the information given seems to vary depending on who you speak to. Just do as much as you can to help yourself ie getting help with the form, sending tracked delivery etc.
