  • I haven't seen the four page form yet, but I don't think I would recevie that one anyway. I fill it in for my son, and as he is under 16 I think that the longer form will be used because children change so much anyway.

    It is a pain having to repeat yourself so often, and in my son's case, I have to fill in every page except the one for blackouts and seizures. I have to fill in each box in detail (day and night in many cases) as I know that he would not get the correct award if I didn't.

    The four page forms sound like a dream in some ways, but I do understand the need for longer forms.
  • how do u know if your area is going to get a 4 page or the normal page ?
  • i posted earlier in the thread that it manchester,east lancs and the north east,i assume for about a year before they decide to to roll out nationally IF its deemed a success

    i suffer bi polar and really dont know how capable i am of filling a kettle when i am ill!!i cycle internally all day with the drugs keeping me within 'normal' bounds-how can i comment on my illness--i am not a psychiatrist! the condition will never change -send me to full time work and cut off my life supporting money and i will get ill--the thought of filling in and worrying about what the assessors think about my oppinion of my illness in itsself is enough to start my illness off again! i have the support of my gp and ex physchiatrist and they do the real asssement and probably answer just as long questionaires about me-it cant change and never will--the new forms sound like a solution to my continual worry-leave it to the docs-manics worry about everything and anything even when we are well!!--whatever happened to life time awards--

    It took me three weeks on and off to fill in my recent IB50, my ability to concentrate can be that low at times. I find the forms exhausting. At least I got four weeks to fill it in and send it back.

    I understand the need for the longer forms, however, once someone has been on DLA and other disability benefits a fair length of time, giving consistent answers year upon year, a shorter form seems much more reasonable.
  • i got the new 4 page form to fill in in december i returned in on the 7th jan and got my new award letter today (29th jan), my award is the same as last time high care and low mobility it was a 3 year award.

    Hi all,just an update,my wifes award was renewed after using the new '580'form.We were very apprehensive about the new format hence this post.The whole procedure took about five weeks.

    Well, I finally returned my form in February (I finally worked out how I was meant to fill it in) and 3 weeks later got a letter saying my DLA has been renewed at the same rate for another 3 years (last award was for 2 years).

    They can't have contacted my doctors or anyone (forgot to give them the details of my new one after moving) so looks like they took my word for it that I needed the same amount of help. If it's like that for everyone then it's a massive improvement.

    I used the 4 page DLA form and after a 4 month wait had my award reduced. If you are filling in the 4 page form and your circumstances haven't changed my advice would be to get as much evidence from the various medics who are treating you as possible and sent this in too - even though the form doesn't ask you to. I'm having to do that now on a reconsideration of their decision.
