New 4 Page DLA Renewal Form( DLA580)

Hi all,
Has anyone received renewal of their DLA after filling in one of the new shortened to 4 pages DLA renewal forms after having no changes in their circumstances.
My wife received hers at the begining of November.Basically if there are no changes to report you simply sign the form and send it back.
The new form seems too good to be true!

Hi, i got my DLA renewed after filling in the 4page document. My circumstances hadnt changed, in fact they will get much worse.

I had DLA renewal forms beginning of October and it was a nightmare - 39 pages! There is no change in my condition but I had to fill it all up again and there were slight changes to some parts/questions. So if there is a new renewal 4 page form, I didn't receive it!0

Sounds amazing,
I am just about to start the horrendous task of renewing my DD's DLA form, but she has gone from a child one to an adult form now that she is almost 16. Anything to make these forms less daunting, gets my vote.0 -

This just makes me want to cry, and just feel more down and depressed! I stressed myself out so much with the last form, and to find this out, I just think it isnt fair to be honest, on the others that have had the stress of filling in the 40 page form!
I am glad though they have this, and at least some others hopefully wont have the stress.0 -
I wish everybody was given the same chance of this new shorter form. I have recently subnitted my renewal form after filling it in with the help of ncodp. My circumstances have got worse since my last claim. I have Fibromyalgia and also Bursitis in both hips which is extremely painful. I, like a lot of you suffer constant pain, usually in silence !. I had been getting middle rate care and high rate mobility but have just had my reward turned down after 6 years. They have now just given me low rate care. I am so depressed about this and am worrying myself silly. My doctor is doing me a letter to take with me next week when I have someone to help me with the appeal form. I only got a mobility car last year and don't know what I will do if it is taken away. I hardly go out now because of the constant pain so I guess I will just have to stay home.

It really makes you wonder how on earth the DLA decision makers can make an award judgement based on a 'no changes' signed declaration on the 4 page form.
I can only imagine a lot more medical assesments will be made in the future if the new form is adopted nationwide.0 -
If the new form is adopted nationwide I suspect it can only be a good thing after all if you just sign a form to say no change then the DM will be going against the last decision if he changes the award.Its worth reading what "rightsnet"has to say on the subject.0
