Advice regarding a pregnancy and failed vasectomy

I'm trying to remember how many consent forms I've signed. It's at least 4 - D&C following miscarriage, sterilisation, brain surgery, coil fitted under GA (because they might have been able to do something else). Not once have I been given a copy to keep - and I know I'd remember if I had a copy of the brain surgery one, which was awesomely scary!
Well I recently had minor surgery, didn't want a copy of the consent form, two people tried to persaude me and left it in the notes , the third person forced me to take it against my will !! Maybe I should sue them for that !! :rotfl::rotfl:

Did you pay for the operation? If not then make sure you subtract the cost of the procedure from anything you might get awarded to make it fair...
I'm sure the chance that it wasn't 100% would have been mentioned - whether your husband took notice or not is a different matter I guess
If he'd ended up with a huge problem because something had been left behind etc then yeah I'd probably have a different view on it - but they didn't leave anything behind and it obviously worked for 2 years (unless you abstained!) so chances are they have "rejoined".DFW Nerd #025DFW no more! yes, I realise that, you just seemed to be a bit dogmatic before that the OP's DH would definitely have been given a copy of his consent form, and should have kept it in case he needed to refer to it.

Again you really can't know this without access to his medical records. You don't know why the operation has failed. He needs to have the reasons for his returning fertility investigated with his doctors and seek formal legal advice if he is interested in claiming.
No qualified lawyer would advise on the limited facts we have at present so it amazes me how many barrack room lawyers feel able to advise in such black and white terms on so little information!0 -

this info i posted was on the nhs website, it's their own words.. i was trying to find info about how effective/not effective it was, no contraceptive is 100% apart from (harsh) a complete hysterectomyLife is about give and take, if you can't give why should you take?0 -
I had my vasectomy in November 2008, and at the moment I've still not been given the all clear yet.
What I can tell you, for certain is that before you have the op (at a meeting that your husband would have attended) they talk through the procedure, book you in for a certain date, and you sign to say that you know it's not a 100% bullet-proof form of contraception.0 -

In the hospital i work in we ensure the patient/parents keep a copy of the consent form, it is a carbon copy form, one for the notes and one for the patient0
