Financial FactSheet Brainstorm Help Required

While filming Make Me Rich, i was staggered at the number of people who didnt' know who their providers were for things. Also in many couples one partner looked after the money and if anything happened to them, the other partner wouldn't know where to begin.

I started telling people to produce a 'financial fact sheet' listing all their providers.

Now I've decided the site should have a nice easy one.

My plan is as follows

1. To have categories and an easy fill in chart
2. To help people llook up their providers website and phone number
3. To allow it to be printable and to store it on the computer, it will probably be done in word, so that all of it is in the users computer rather than online so there are no data protection or security issues

Therefore i have the following questions

a. What info should be in it - i am planning not to include account details as that's a security risk, would it still be useful in that case

b. I currently (having done it quickly) have the folllowing categories, im sure im missing many. Any suggestions

Credit Cards

Savings
Cash ISA
Bank Accounts
Telephone Providers
Internet Provider
Current Accounts
Shares/Investments
Gas Provider
Elec Provider
Water Bill
Car Insurance
Roadside recovery company
Car Servicing company details
Home Insurance
Pet Insurance
Travel Insurance
Pension providers
Annuity providers
Mortgage Companies
Life Assurance Companies

Loan Company
Health Insurance Provider
Healthcare Cashback Provider
Other insurance providers
Childcare Voucher Provider
Child Trust Fund Account
Any HP deals

Do you have a will, if so location
National Insurance Number
Passport Number
Nearest Locksmith (how to find)

c. Any other thoughts of what'd be useful

If they have their own business, financial details of that and what to do with it would be good.

It's hard to find decent tradesmen who do a good job and don't charge over the odds. Once you find one though, it's easy to lose their details. I'd include plumber, roofer, plasterer etc on mine.
May all your dots fall silently to the ground.
