All you really have to do is list the details of the dd and ask them to cancel as below
Payee A N Other
Amount £20.00 monthly on 3rd of month
From account Your bank details would go here
Please cancel the above direct debit as of (date) and ensure no more payments are made from my account.

When you fill out your bank details at the bottom it says about it being your a/c etc. and there is a box to check. If you don't check the box but just press continue it lists your order details and asks you to submit. Once submitted its puts a message up saying subscripition successful. It doesn't get you to print DD form.0 -
