Número de teléfono que se va a recargar=Number of your phone
Número Tarjeta=Credit card number
Fecha Caducidad=Valid to date
Banco o entidad a la que pertenece la tarjeta=Name of credit card,ie Bacarclaycard,Halifax etc
Then click Recarga when all is filled in!!

12 + 1 month on Amena
