This is interesting to hear @Mr_Spanky . We are currently going through this process, and have had half of the money refunded, same as your experience, but can't get a complete refund. We are hoping the bank will help, but their policy is only to chase disputes over the last year, so we are stuck for earlier payments.
Do you think it was the threat of court action - did you mention anything specific that might be of help to us?

I just can’t get my head around this post.

Who doesn’t check their bank account every week, month etc, I check my 5 bank accounts daily, takes 2 or 3 minutes, I would know if a penny was missing, 15 direct debits, 3 per account, at one point 21 regular savers, so another 10-15 accounts, transfers of 20k - 50k most months. The last time a had an issue like this was with Orange mobile phones, pre 2000.

Yep it boggles my mind. Throwing away hard earned money. The only way I give it up is out of my cold dead hand.
