claim from Euro car park , HM court claiming £170

I hope you are all well .

I have just received the General form of Judgement or order. Re Kingston upon Thames county court

I have been requested to to submit a witness statements no later than 28th May 2026. Do I have to sent them to the court address and copying Euro car park to their office address ?

Whom to address the witness statement to the judge ?

By the way do you have a copy of witness statement that I can tailored to my case please.

I paid the parking ticket but the number plate was not entered correctly and Euro Car park has admitted to have received the money but still wanted to issue a ticket

I am a bit worried if i made a mistake on the statement, the Judge will judge against me .

Can anyone advise how to sent my witness statement to Kingston Upon Thames County court by post or email ( please do let me have the email address if you have it ) there are no email address on my General form of judgement or order.
