    You can always do a Subject or Data access request.. to the bank and Equniti (the would have transfer your data to the current organisation. ICO had said that there is no law to say how far back they need to store your data for but no harm trying.They have to supply all your data within 30 days but if you be specific about what data you need it can take less time but no guarantee. No need to mention you have misplaced your share holder number because more hoops to jump through, they may ask for it- as it will help them to do less work but they should still be able to find your records.

    Dear Data Protection Officer

    RE: Subject access request / <Your full name>
    Policy number xxx < if you do not know it leave this out>

    Please supply all the personal data you hold about me, which I am entitled to receive under data protection law, to present. If you need any more information, please let me know as soon as possible.

    I would prefer to receive the <data in printed out and voice recorded format.> Please change this if you want it to be emailed to you

    It may be helpful for you to know that data protection law requires you to respond to a request for personal data within one calendar month.

    If you do not normally deal with these requests, please pass this letter to your data protection officer or relevant staff member.

    If you need advice on dealing with this request, the Information Commissioner’s Office can assist you. Its website is ico.org.uk, or it can be contacted on 0303 123 1113.

    Yours faithfully,
    <YOUR NAME>
