Can't transfer ISA from Monument Bank to Castle Trust Bank

Has anyone else had issues transferring their ISA from Monument or to Castle Trust? Both me and my husband are having issues and both banks are blaming each other. Castle Trust are saying that Monument keep rejecting the transfer request and Monument are saying that they have not received any requests. I don't know who to…
