A friend from South America was coming to visit us and applied for a visa.
She received her passport with no visa and no explanation.
Does this mean that she has been rejected?
Her husband and son have been accepted (so no idea why she was rejected although she did come her last year without them).

This is probably a clerical error: best if you write to your MP giving full details, and at the same time ask her to email your MP and give him/her permission to investigate the problem, quoting your address.1

Hasn't she contacted them to ask?

Whereabouts is she from? South America isn’t a country and most of the countries in S America don’t need a visa (but do need an ETA).But if she does need a visitor visa then yes, it sounds like an error. If UKVi refuse a visa then they’ll let you know.1
