The question was just about the ESA report and GPFR, I just wanted to ask if it would be enough to mention them on the PIP review form along with the fact I had a paper based assessment for ESA, I'm very reluctant to request the report and GPFR, it's making me feel anxious because I really don't want to see what's written in them due to personal reasons.
I do understand that they could help if I did send copies, but I'd have to read them before sending them (reading is a chore on it's own for me).

I'm not sure what to advise... except the timing is issue... obviously ideally any supporting evidence is sent and referenced with the Review form because you want to use your evidence in supporting what you are saying. I'd be inclined to request the documents and try to have a read with a view they could be useful if you needed to challenge the PIP decision... in reconsideration and potential later appeal. I've had GPFRs that were both incredibly helpful and very unhelpful in my time although none have been requested in recent years... so I used them as I saw fit along with any other evidence and rejected anything unhelpful/irrelevant.

I've since been moved to UC LCWRA but this was at the end of last year, same as my review, I'm assuming that I can still use the report and GPFR?

I received my ESA report but after reading it, I'm not going to send it.
It's very short, I'm assuming that's normal? It was a paper based assessment. I doubt it'd help much due to the lack of info.
The report annoyed me a bit and made my anxiety worse which is why I didn't want to request it. I've put it away now.
Although it was acknowledged that it would be a substantial risk the person then puts that they advise that I could be considered for work within 18 months which is really quite silly considering my health conditions haven't gotten any better and they aren't likely to, it's almost like they didn't bother to read the questionnaire as I don't see anything in the report relating to anything in my questionnaire.
Also, they mention the GP, who I'd only seen the once, while she did give accurate info, they are saying that she has said I have suicidal thoughts due to pain which is wrong and I have no idea why she would have said that IF she did because when I spoke to her I never mentioned my FM or pain related to it, I spoke to her about my mental health and heart palps, I briefly told her about traumatic events that I went through as a child and adult (not all), the person even says the GP said that I have a fragile state of mind and that I would unlikely to be able to engage with work or work related activities... It's confusing. My thoughts are related my my traumatic events, not the pain, same as my conditions in general.
I did end up making a complaint about the same GP for certain things but I'm wondering if it's the HCP not reading the GPFR properly rather than the GP saying that. I just don't understand why she would say that.
Pardon the moan/rant, but I'm very stressed and anxious, I've had various things going on here and on top of that I'm worrying about the stuff to do with the government.0 -
When they send an extension letter how long is the extension for?. Mine finishes May 25th but I haven't had a letter and today I received a text message saying "we still have your form". Thanks.0
