Bear in mind that as well as knowing the applicant for >2 years the countersignator must be well aware of their full name, with enough confidence to make their declaration, the purpose being to exclude (or reduce) the possibility that it's a fraudulent passport application.
They must be willing to make the declaration ultimately... we all got to know one of our barmaid's surnames last year when a retired-copper/regular agreed to sign her passport. All known her for years just never had the need to ask her surname.

Yes, in such a case, probably not uncommon, the signatory would have confidence from the years of acquaintance with the barmaid that she's applying for her own passport and not as an imposter.
