Yes it will be a separate word document. One question I have is, I don't actually know the issue of proceedings date.

Just leave that date out.
This can be removed because the BPA CoP isn't dependant on 12 months having passed:"as more than 12 months have passed since the PCN on 24th March 2022 and the CCJ on 18th April 2023."
Have you linked snd quoted the right clause from the BPA CoP? The Judge won't know.
PRIVATE 'PCN'? DON'T PAY BUT DON'T IGNORE IT (except N.Ireland).

You could add to the 4th point in the Draft Order 'plus the Defendant's attendance costs at the application hearing'.PRIVATE 'PCN'? DON'T PAY BUT DON'T IGNORE IT (except N.Ireland).
