    That DQ was sent to you on the 5th April. That was months ago. How come you are only now filling it in? How did you submit your defence and when did you AoS the claim?

    Is the General Sanctions Order against you for having failed to comply with a court order? Possibly because you didn't return the DQ in time? Has the Claimant obtained a default CCJ against you already?

    This is no biggie.

    Just email it today (copy in the Claimant's legal team) and make sure you get an auto-reply from the CCBC.

    Looks like the CCBC is doing a sweep of outstanding DQ cases this month.  I hope we don't get any more repeat threads.
